In preparation for your exam, please go to the FAA web site, https://medxpress.faa.gov/, and complete the online form. Upon completion of the 8500-8 form, you will be provided a confirmation number. Please bring this with you to your appointment. Please remember to bring your valid drivers license and/or passport, contact lens case and solution, glasses, Special Issuances and/or F. A. A. SODA Certificates with you at the time of your appointment. Having all of this will expedite your physical. Should you have any questions.
